How far away should your computer screen be from your eyes?

I have noticed that when I get really wrapped up in what I am working on, I will bring my face extremely close to my computer screen, maybe 9 inches away. I seem to do the same thing with books. How far away should your computer screen be? Same for books. And, how can you train yourself to keep some distance!
